APPLICATION FORMS

OPTION 1) Please return your registration by mail with your cheque or money order to the following address:

OPTION 2) complete the Online form and pay by VISA

Your proof of residency and a copy of the player's medical card will have to be either sent by regular mail or sent as an attachment in a separate e-mail. An additional on-line Creditcard processing fee will be added to the cost of registration.

DOCUMENTS REQUIRED

Registration is complete when the NDG Minor Hockey Association has in its possession:

• The completed form duly signed by one of the parents or guardian or completed on-line.
• A photocopy of the player's Medicare Card and a proof of residency
  (Recent copy of Hydro Quebec, Gaz Métropolitain, Bell Canada or Vidéotron bill)
• Payment in full by Cheque, Money Order or VISA.
  
  All registrations must be received by June 30, 2011

Proof of Residency

NDG Hockey is restricted to players who reside in NDG and Cote des Neiges district. The proof of residency has to be provided with the registration form. Any false representations will result in the player being expelled (with no refund) from NDG Hockey.

AGE CLASSIFICATION

AGE	For the 2011-2012 season, any player born between January 1st and December 31st of one of the years below is eligible to play in the corresponding division:	DIVISION

(1) 4 years old- Any organization can register players born in 2007 in the initiation division
(2) Number of players og 21 years of age allowed to play junior hockey (regional). See article 5.7.3.

Late Registration (After June 30th, 2011)

Late registrations will be accepted only if there is space available. A late registration fee of $25 will be charged.

Training Camp

Players will be called in for the training camp by order of registration. Team levels will be formed on a first come first served basis. Atom camp starts during the first week of September so register now!

Reimbursement

There will be no refund after the first session on the ice.
